The following policies are for the San Jose and Cupertino location only.

Fee Payment

All fees are payable in full at the time of each appointment.

Payments for all phone consultations and missed appointments are due within 1 week after the date of the appointment. A $20 late fee will apply for all invoices 14 days past due. A charge of $25 will be added for all returned checks.

Cancellation and no-show policy.

A charge of $60 will be billed for all missed appointments or appointments cancelled with less than 24 hours notice.

Phone Consultations

Phone calls longer than 15 minutes are billed according to the fee schedule. There is an additional charge for all phone calls returned on a weekend.
